Nasty optometrist

I saw an optometrist because my M.D. said I may have cataracts in the future. The optometrist prescribed two pairs of glasses: 1 for driving and 1 for close work. He said my eyes were too bad for bifocals.

He said I should not be driving at all without glasses. He said I'll need cataract surgery in 3 or 4 years.

Since I had been symptom-free except that I read with drugstore glasses, I got a second opinion. I was legal to drive, but would be 20/20 or better with glasses. 'Drugstore' glasses were okay for close work. My cataracts may need surgery in 14 to 15 years, if then. I got bifocals. I had my eyes tested by Motor Vehicles agent, both bare eyes and with my new bifocals. MV says I'm okay to drive without glasses, but I'm 20/20 with.

Now the original optometrist is warning the second one that she can be sued if I have an accident driving without glasses.

I want to write a letter to the 'good' optometrist saying that

1. I am solely responsible for my decision, and
2. neither I nor my heirs will ever sue her if I should ever have an accident.

How should I word that letter?
